• Transportation to and from the center (subject to Insurance and plan guidelines)

• Safe and reliable transport coordinated with care plans

Social Adult Day Care provides a structured, non-medical daytime program for older adults who benefit from supervision, social engagement, and supportive care in a welcoming community setting. The program is designed to enhance quality of life, promote independence, and reduce social isolation-while offering families peace of mind during the day.

Care Coordination & Family Support

Who Is Eligible

• Individuals enrolled in Insurance

• Participants approved through their Insurance or managed care plan

• Eligibility is determined based on individual needs and program availability

A Safe, Non-Medical Program

Social Adult Day Care focuses on social support and supervision. It does not provide medical, nursing, or clinical services and does not replace primary care, home care, or emergency services.

For medical emergencies, participants should call 911.

Coverage & Compliance

Social Adult Day Care services are provided in accordance with New York State Department of Health guidelines. Coverage, scope of services, and availability are subject to Insurance and managed care plan authorization.
